When you choose a Primary Care Provider (PCP), it means that you are establishing a one-on-one point of care provider who will treat you with respect and coordinate all of your healthcare needs. This relationship requires that you fulfill the obligation of scheduling and keeping well child exams for your child(ren) and keeping your scheduled follow-up visits. You, your family and NAG Clinics are in a relationship.

What is a PCP?

A PCP, also known as a Primary Care Physician, is the primary care provider for the healthcare needs for your child(ren). The PCP is responsible for providing the basic medical care needs, such as well-child examinations and immunizations, and other basic medical, behavioral, or social needs. Additionally, the PCP is responsible for coordinating your specialty care needs with specialist, such as dermatologist, allergist, and orthopedists.

What is a Patient-Centered Medical Home?

A Patient-Centered Medical Home is a care delivery model whereby patient treatment is coordinated through their primary care physician to ensure that the patient receives the necessary care when and where they need it and in a manner that the patient can understand. The Patient Center Medical Home will also cut down on the need for you to seek care in Emergency Rooms for primary care medical conditions.
